Ochsner Care Management Programs

Ochsner Health Plan has special programs to help you stay healthy and manage your care. These programs are free and you can choose to join or not. Here are the main programs:

Transitions of Care Program:
This program helps you when you leave the hospital. It makes sure you have everything you need, like doctor visits, medicine, and support at home. The goal is to keep you from going back to the hospital and to make you feel better.

General Case Management Program:
This program helps you with your health by giving you information and support. A care manager works with you to make a plan that fits your needs. They help you get the services and benefits you need.

Complex Case Management Program:
This program is for people with serious health problems. It helps you get the right care and services. The goal is to help you get better and stay healthy. You can choose to join or leave this program anytime.

Dual Eligible Special Needs Program (DSNP):
This program is for people who have both Medicare and Medicaid. It gives extra help and support. You will have a care manager and a team to help you with your health needs.
