This directory provides a list of Ochsner Health Plan’s network providers.

This directory provides a list of Ochsner Health Plan’s current network providers for Ascension, East Baton Rouge, East Feliciana, Iberville, Jefferson, Lafourche, Livingston, Orleans, St. Charles, St. John the Baptist, St. Tammany, and West Baton Rouge parishes in Louisiana. To request a hard copy of Ochsner Health Plan’s provider directory, please call our Member Services Department at 1-833-674-2112. We are open 8:00 a.m. to 8:00 p.m. seven days a week from October 1st to March 31st and 8:00 a.m. to 8:00 p.m. Monday through Friday, April 1st to September 30th. Ochsner Health Plan will mail a hard copy of the provider directory to you within three (3) business days of your request. Ochsner Health Plan may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently. If you request it, your request for hard copies of the provider directory remains until you leave Ochsner Health Plan or request that hard copies be discontinued. Please contact Ochsner Health Plan if you need information in an accessible format (braille, large print, etc.). Call Member Services at 1-833-674-2112. We are open 8:00 a.m. to 8:00 p.m. seven days a week from October 1st to March 31st and 8:00 a.m. to 8:00 p.m. Monday through Friday, April 1st to September 30th. TTY users should call 711.

Members should use this directory to identify in-network providers. To select or change your PCP, please call Member Services at 1-833-674-2112. Hours are 8:00 a.m. to 8:00 p.m. seven days a week from October 1st to March 31st and 8:00 a.m. to 8:00 p.m. Monday through Friday April 1st to September 30th. TTY users should call 711.

You will have to choose one of our network providers listed in this directory to be your Primary Care Provider (PCP). Generally, you must get your health care services from your PCP.

Your PCP is your main contact for health care. Your relationship with your PCP is an important one because your PCP is responsible for the coordination of your health care and is also responsible for your routine health care needs. It is important for you to develop and maintain a relationship with your PCP. Your PCP can offer most of your care. Examples of PCPs are internal medicine, general practice, geriatric, and family practice doctors.

If you need specialist care or hospital services, your PCP can help set up services for you. You can also set up services yourself by getting care from any provider in our network.

The network providers listed in this directory have agreed to provide you with your health care services. You may go to any of our network providers listed in this directory. Other providers are available in our network. Ochsner Health Plan Freedom members may seek care from an out-of-network provider at a higher out-of-­pocket cost.

Ochsner Health Plan Premier (HMO) has a network of doctors, hospitals, pharmacies, and other providers. If you use the providers that are not in our network, the plan may not pay for these services. You must use network providers except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers, neither Medicare nor Ochsner Health Plan will be responsible for the costs.

Ochsner Health Plan Freedom is an (HMO-POS) plan, Members may use Point-of-Service (POS) providers that are outside of our network for an additional cost. Out-of-network/non-contracted providers are under no obligation to treat Ochsner Health Plan Freedom members, except in emergency situations. Please call our member services number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.

All Ochsner Health Plan Members may obtain emergency services from the closest available provider and may obtain urgently needed services from any qualified provider when out of the plan’s service area or when network providers are unavailable.

If you receive a bill from any out-of-network provider, you should not pay the bill but should submit it to Ochsner Health Plan for processing and determination of your liability, if any.

Ochsner Health Plan makes every effort to ensure that this list of providers is up to date and accurate. Information listed in this directory is not guaranteed and may be subject to change without notice. Revisions to this directory may not be made immediately. For the most up-to-date information, please verify that the provider you have selected is covered by your plan. You may do this by contacting member services at 1-833-674-2112  (TTY: 711) or by calling the provider prior to scheduling an appointment to verify that he or she is covered by your plan and is accepting new patients. The provider network may change at any time. You will receive notice when necessary.
