We are pleased to introduce a new mobile app and rewards program to help you develop and maintain healthy habits — and earn rewards along the way!
The Vitality program has tools and resources to help you stay dedicated to your health routines. You can achieve long-term results and earn points toward a higher status by staying engaged and completing activities.
Certain healthcare activities are even rewarded with a Walmart gift card!
You will need to prove that you have completed these activities in order to earn your reward. See the Frequently Asked Questions and Rewards chart on the following pages for more information.

To get started, download the Power of Vitality app from your phone’s app store and follow the prompts to complete your registration.
PERSONALIZE YOUR EXPERIENCE
Complete the onboarding assessment to earn your first points and personalize the program to your current lifestyle habits and future health goals.
LINK A FITNESS DEVICE OR APP
You can earn up to 15 points per day for completing verified workouts. To link a compatible fitness device or app to Power of Vitality, navigate to Profile > Apps and devices.
EXPLORE & FIND YOUR FOCUS
Explore your Power of Vitality homepage and navigation bar to get familiar with the learning and point-earning opportunities available to you. The ‘Focus on your health’ and ‘Earn points’ sections are great places to start!
Frequently Asked Questions
Q: How do I complete the rewards activities receive a gift card?
A: To complete an activity that is eligible for a gift card, log into your Power of Vitality account. You will find the annual wellness visit activity within the prevention category and the Ochsner Health Plan (OHP) health risk assessment activity within the OHP health risk assessment category at the bottom of the homepage.
Q: What type of gift card will I receive for completing the activity?
A: You will earn a Walmart gift card for completing eligible activities. These cards are redeemable at Walmart and on Walmart.com, and cannot be used to purchase alcohol, tobacco, firearms, or lottery items.
Q: How and when will I receive my gift card?
A: If you participate in the Vitality platform via website or app, you will receive your gift card to the email address used when creating your Vitality account. Your gift card will be available within one business day of the activity showing as completed within your Vitality account.
A: If you request to be an offline user, you will receive your gift card to your home address within five business days of the activity showing as completed within your Vitality account.
Q: I don’t have a computer or a smartphone, how do I participate in the program to complete activities and earn a gift card?
A: You can still participate in the Vitality program by calling Vitality Customer Care at: 1-877-224-7117 and requesting an offline packet.
Q: Why are there two health assessment activities within the program?
A: There is an OHP Health Risk Assessment (HRA) and a separate Vitality health assessment. For 2025, completion of only the OHP HRA will earn you a $20 gift card. To better tailor your member experience within the Vitality program, Vitality uses responses from their health assessment. This information allows the Vitality program to learn more about you and recommend healthy activities.
Q: How do I complete the Health Risk Assessment (HRA)?
A: You can call an Ochsner Health Plan (OHP) Care Manager at 504-300-8760 to complete the health risk assessment. Completion of the survey is automatically tracked, and points will be awarded within 45 days.
