Ochsner Health Plan ended its Medicare Advantage contract on December 31, 2025. No new prior authorization requests, concurrent review requests, or inpatient admission notifications will be accepted for services dated on or after January 1, 2026. Providers should direct all 2026 coverage and authorization inquiries to the member’s new health plan. Information on this page applies only to claims, appeals, and services rendered on or before December 31, 2025.

Durable Medical Equipment Providers (DME)

Ochsner Health Plan Freedom and Heroes (HMO POS) members may use Point-of-Service (POS) providers that are outside of our network for an additional cost. Out-of-network/noncontracted providers are under no obligation to treat Ochsner Health Plan members, except in emergency situations. Please call our member services number or see your Evidence of Coverage for more information, including the cost-sharing that applies to out-of-network services.
